Below you will find the policies & procedures for various situations and activities that may be required or occur while participating in athletic activities for West Central Middle/High School.  If you have any questions pertaining to one of these policies or procedures  below or have a question about a situation that is not described below please contact the athletic department at (219) 567-2534.

Online Student – Athlete Registration for Sports Participation  

We are very excited to announce that the West Central High School Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

We are asking that ALL parents of athletes use FinalForms to register your students for Athletic participation at West Central Middle/ High School.

IHSAA Pre-participation Physical and Health History Update Questionnaire and Consent and Release Certificate.

All student-athletes are required each year to have a physical prior to participating in any West Central sport-related activity (conditioning, open facility, weights, practice) by Monday, July 6, 2020.  In addition, due to the COVID-19 pandemic, an updated Health History Questionnaire and Consent and Release Certificate is required for participation.  Downloadable forms are below but electronic signatures will be required via your Final Forms account prior to July 6th.

Student-Athlete Travel Release and Overnight Trip Policy

In the event a student-athlete requests to travel from an athletic event with a family member, the student-athlete, coach, and legal guardian must sign and submit a travel release form to the athletic department.  A downloadable form is below.

Concussion Management and Sudden Cardiac Arrest Acknowledgement 

The Indiana state legislature and Indiana Department of Education require that all parents and students receive educational materials on Concussion recognition, treatment and return to play criteria. This information must be read by both student and parent, and a concussion acknowledgement form must be signed and on file in the Memorial athletic office prior to beginning sports activity each year.  Over the past several years concussion awareness has become a very important topic in athletics, especially at the high school level.
